Most doctors think the basics are obvious.

They are not.

Today’s episode is about SOPs and training, and why they quietly break down inside growing practices.

I reviewed five client accounts today. Every one of them had the same issues. Poor phone conversion, weak block scheduling, low treatment acceptance, and sloppy recare rates. These are not small problems.

They control the health of the practice.

This is not because the teams are lazy or incapable. These were good people who wanted to do well.

The real problem was leadership absence.

The doctors were busy doing dentistry.

Production was happening. Patients were being seen. But systems were not being taught, reinforced, or inspected. When leadership steps back, standards drift. What feels obvious to the doctor never becomes obvious to the team.

SOPs only work when they are trained, repeated, and enforced. Accountability only exists when someone owns it. If the leader is not present in the systems, the systems decay.

This episode is a hard look at why good practices stall, and why the fix is rarely hiring better people. It is almost always stepping back into leadership where it was quietly abandoned.
